Combining services in respiratory care in central and south Manchester
Future hospitals COPD programme Manchester
The North West currently has high smoking rates and very poor outcomes for respiratory disease with high levels of social deprivation, mortality and morbidity. Historically, respiratory services across central and south Manchester have been fragmented with barriers between primary and secondary care and also between Central and South Clinical Commissioning Groups’ (CCG) geographical boundaries. The Manchester Respiratory Future Hospital Pilot (MRFHP) aims to combine the services to allow patients to access the same high-quality care regardless of postcode. The creation of an efficient service will make best use of financial and workforce constraints, whilst delivering patient-centred care which is closer to home.
The key messages of the film will be why this work is important, how we have involved patients in leading change and how we have built relationships across multiple large organisations to transform care.
